In an effort to gather accurate information regarding this provision, the Department of Human Services (DHS) developed an attestation form. For each nursing facility to receive credit for participating in sessions attended by staff, a form must be completed and submitted to the Department no later than this Sunday, February 12, 2023 for the sessions attended in 2022.
The Department distributed this form in an OLTL nursing facility ListServ. If your organization has not submitted the required Attestation Form to qualify to receive payment, PHCA encourages nursing facilities to fill and submit them to RA-PWNFLNATTEST@pa.gov as soon as possible by downloading and completing the form here: LN_NF_Attestation Form V1.0_1-12-2023
