Takeaway Document
PHCA is requiring each speaker to provide attendees with a one-page takeaway document that attendees can utilize to implement your recommendations and share with their staff. This initiative is a result of attendee feedback from prior Summits, where attendees valued the education but felt overwhelmed in taking that first step toward implementing what they learned.
Please complete the form below by no later than Friday, March 2. Your responses will be used to create the takeaway document. Hard copies of each takeaway will be provided in attendee packets. If you would like to view your takeaway document prior to the Summit, please request a copy by contacting Wendy Johnson at wjohnson@phca.org.
