The implementation of the Community Health Choices (CHC) program promises historic change for long-term and post-acute care providers in Pennsylvania. Since January 1st of this year, your colleagues in the 14 county Phase I zone have been operating under CHC. This webinar will allow you to learn from your colleagues that operate nursing facilities in the Phase I zone of CHC. The presenters will share the steps they took to prepare for implementation and some of the challenges and successes they experienced. Presenters will offer insights regarding contracting, claims processing, working with service coordinators, transportation services, participant enrollment and eligibility, working with the IEB, and other issues they have faced under the CHC program.
Learning Objectives:
- Learn about best practices that can assist in a nursing facility’s success under CHC.
- Learn how to successfully work with the CHC-MCO plans and their service coordinators to the benefit of the residents.
- Learn about the resources available to nursing facilities that are in CHC Phase II zone and the steps to implement to address challenges.
