Webinar – CHC Lessons Learned

The implementation of the Community Health Choices (CHC) program has brought historic change for long-term and post-acute care providers in Pennsylvania. Since January 1, 2018, your colleagues in the counties affected by Phase I and Phase 2 have been operating under CHC. This webinar will allow you to learn from your colleagues that operate nursing facilities in these zones of CHC. Having assisted ten plus providers through contracting and implementation, the presenter will share the steps providers took to prepare for implementation, and some of the challenges and successes they experienced. Presenter will offer insights regarding contracting, claims processing, working with service coordinators, transportation services, participant enrollment and eligibility, working with the IEB, and other issues they faced under the CHC program.

Learning Objectives:

  1. Learning about best practices that can assist in a nursing facility’s success in contracting for CHC.
  2. Learning how to successfully work with the CHC-MCO plans and their service coordinators to the benefit of the residents.
  3. Learning about the resources available to nursing facilities that are in CHC Phase III zone and the steps to implement to address challenges.
