C-6: Health Information Exchange Participation: A Provider Case Study

This session will provide a brief overview of HealthShare Exchange (HSX) and introduction to Health Information Exchange (HIE). HSX will highlight a case study of a post-acute provider organization. This case study will show how this post-acute provider engaged with HSX. HSX will highlight the specific needs of this provider organization and what services are utilized to help improve care coordination, quality outcomes and any impact to reimbursement. The session will highlight the benefits of data exchange and how to maximize your organization’s electronic health record for this information. Session will include information on privacy best practices, consent management and technical implementation requirements.
